End-of-Life Planning & Death Doula
Consultation Package
Purpose of the Consultation
Death is hard for all involved. But it can also be a peaceful, sacred time, filled with love and fulfillment. As your End of Life and Death Doula, I will be your guardian to a peaceful passing.

The consultation is a supportive, non-medical conversation designed to understand your needs, values, and concerns around end-of-life planning, serious illness, or death care. This is a space for clarity, emotional support, and practical guidance—at your pace.

Intake & Consultation Questions — End-of-Life and Death Doula Practice (Alberta)
A. Opening the Conversation
Purpose: build trust, explain role, and set expectations
What made you call me today, and what are you hoping support from a death doula will look like?
Have you worked with a doula or similar support person before?
What are your top priorities or concerns as you think about this stage of life?
Is there a preferred way for you to communicate (e.g., gentle, direct, spiritual language, practical focus)?
How did you hear about my services?
Understanding the Situation & Care Context
Are you seeking support for yourself, or on behalf of someone else?
Where is the person currently receiving care (home, hospice, hospital, continuing care)?
– If in hospice/palliative care, how has communication with the care team been?
What medical diagnoses or prognoses (if any) have been shared with you or your loved one?
Note: Death doulas in Alberta are unregulated practitioners — I provide non-medical, non-legal support, emotional presence, communication facilitation, and planning guidance, but I do not diagnose, or prescribe, medications. I will use plant therapy including essential oils, topicals and diffusers as well as personal care, comfort, companionship and education. We can tailor your services to meet you where you need and adjust as you desire. I understand things can change quickly and I will adjust as you see fit.
